Online Treatment Forms
Guest consent and treatment intake forms are necessary to ensure the best results for each treatment at each session. To optimize your time, both forms must be completed and/or reviewed prior to each service once your appointment has been confirmed.
Please complete intake forms no more than 24 hours prior to your scheduled service.

Thank you for taking the next step to better, hydrated skin! Your reservation request must be accepted to confirm the appointment. Upon confirmation you'll receive an email/text with links to complete the waivers. To maximize your treatment time, please complete the waivers and intake form BEFORE the appointment. You may want to discontinue use of physical and/or chemical exfoliants and be mindful that certain medications or topicals are contraindications. Now, show up and RUDO Studio will take it from there.

A little something called transepidermal water loss (TEWL). Scientifically speaking, TEWL is:
"the quantity of condensed water that diffuses across a fixed area of stratum corneum to the skin surface per unit time."
In other words, how much and how quickly water passively evaporates through the skin and into the air. TEWL is natural but retention can be managed to enhance skin integrity. TEWL is greater in Black and Asian skin. Other contributing factors are sweat gland activity and skin temperature. This leads to sensitivity, dehydration, and can make skincare treatments less effective. The lower the TEWL the more irritants and bacteria are kept out.

The terms clean, green, and sustainable are terms with no government regulations, standards or classification. However, the terms "clean" and "green" originated from eating lifestyles. Clean products are thought to be free of toxins, allergens, carcinogens, and hormone disruptors (e.g. phthalates, sulfates, parabens). Green product formulations are mostly plant-based and may also be labeled vegan and/or cruelty-free. Sustainable emphasizes environmentally-friendly packaging and manufacturing practices. These terms gave way to “conscious” beauty.

RUDO Studio® e-gift cards may be used for face and body treatment services only. Treat eGift cards like cash. It is not redeemable, exchangeable nor refundable for cash. Lost or stolen eGift cards will not be replaced. The card carries no expiration date. If card is not redeemed within 90 days from the issue date, a $2 service fee per month thereafter will be applied upon redemption.

A minimum of 24 hours advance notice is required to cancel or reschedule an appointment. This allows the opportunity for a prospective guest to make a reservation. If you give notice less than 24 hours in advance you will be charged a minimum of 35% of the full amount of your scheduled service. This amount must be paid prior to scheduling your next service. Two (2) or more same-day cancellations will require a 50% nonrefundable deposit to rebook.

Any person who fails to give advance notice of cancellation/reschedule, forgets or consciously chooses to forgo their appointment for whatever reason will be considered a “no-show.” No-shows will be charged 35% of the full service cost for the scheduled service(s) via the card on file and must be paid.

If you arrive late, your session may be shortened to accommodate any appointments following yours. RUDO Studio will determine if there's enough time remaining. Regardless of the treatment length, you will be responsible for the cost of a full session.

RUDO Studio desires to be reasonable and fair to all guests recognizing unanticipated events happen. RUDO Studio reserves the right in its sole discretion to suspend guests from current and/or future services, for any reason including but not limited to recurring cancellations, no-shows, and/or late arrivals.
